Radbrook Green Surgery

Welcome to our surgery website – we hope you find it useful

Radbrook Green will commence Online Appointment Requests from July 14th

All requests to see or speak to a Clinician are to be made by completing the quick and easy online triage form below (Get in touch online). Any patient’s who do not have access to the internet or are unable to complete the form themselves can continue to contact the practice who will complete the form on their behalf.

The online triage form is available Monday – Friday between 8.00 am – 5.00 pm. The form is simple to complete, please try to give us as much information as possible.

All requests are reviewed by a member of the Clinical Triage Team. If your request is urgent, the Clinician will request a review that day. The Clinician may decide the request is less urgent based on their clinical guidance and you will be contacted accordingly.
