If you live within our Practice area you are welcome to register with us.

Non-urgent advice: Please Note

Patients register with the practice rather than with a particular doctor. 

If you do have a preference of which GP you see, this can be recorded when you register. 

Before registering please complete a New Patient Registration Form and a New Patient Questionnaire

When you visit the surgery for the first time you will be asked to sign them to confirm that the details are correct


Communication

The Accessible Information Standard (AIS) was introduced by the government in 2016 to make sure that people with a disability or sensory loss are given information in a way they can understand.

It is now the law for the NHS and adult social care services to comply with AIS.

Radbrook Green Surgery is working to improve how we communicate with patients.

Proxy Access

There are a number of patients who book their appointments, request repeat medication and can view certain parts of their medical records online.

Patient Access have now improved their system to allow parents and Carers to be authorised to all of the above-mentioned functions for children or the people they care for.

Any patient or Carer wishing to have proxy access will need to sign up to this service by completing a form.
