This is a guide to the General Practitioners within the partnership as required by the Freedom of Information Act 2000. Introduction This Publication Scheme is a complete guide to the information routinely made available to the public by General Practitioners. It is a description of the information about our General Practitioners and the Practice which we make publicly available. It will be reviewed at regular intervals and we will monitor its effectiveness. The partners in the Radbrook Green Practice are: Dr. Simon C Reid Dr. Russell works as a full-time
GP but some of the other partners are involved in other aspects of Health

for information from the Scheme, please write to: CLASSES OF INFORMATION All information at the Radbrook Green General Practice is held, retained and destroyed in accordance with NHS guidelines. Our commitment to publish information excludes any information which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are the protection of commercial interests and personal information under the Data Protection Act 1998. This applies to all Classes within the Publication Scheme. The information on this Scheme is grouped into the following broad categories (click on the titles to skip to the relevant section or simply scroll through): In addition to the five partners, the following doctors are employed in the Practice: Dr. Charlotte Hart who is a
part-time GP Usually there is also a Registrar GP working in the Practice. A registrar is a fully qualified doctor who is undergoing additional training in preparation to taking up an appointment as a General Practitioner. There are three Practice Nurses, Sister Valerie Bowen, Sister Michele Davies and Sister Emma Fewtrell. Additionally working in the treatment rooms are two Health Care Assistants, Mrs Heidi Pritchard and Mrs Sandra Frost. There is a Practice Manager and 16 further administrative staff, plus three cleaners. The Practice Manager is Mr Richard Jewsbury, the Office Manager is Mrs. Carol Shields and the Reception Manager is Mrs Jane Jones. The Practice is a member of the Shropshire County Primary Care Trust whose web site is www.shropshirepct.nhs.uk where a full list of other local General Practices can be found with a wide range of other information. The Practice aims to follow National Institute for Clinical Excellence and National Service Framework Guidelines. (Up) The Practice provides a full range
of Primary Care General Medical Services under contract to the NHS
including: A Mental Health Counsellor holds regular clinics in the Surgery. Although not part of the Practice, the Health Visitor and District Nurse teams work from the surgery and maintain close working relationships with the Practice doctors and staff. The Practice also provides services under its contract with the PCT for Temporary Residents and the partners are all members of the Shropshire Doctors' Co-operative ('Shropdoc') which provides medical services outside normal working hours. Home visits are available to patients who are housebound or too ill to attend the Surgery. Telephone advice can be given by doctors and telephone consultations can be arranged through reception in the normal way. The Surgery is open as follows:
A range of non NHS services are provided on the payment of a fee. Current tariffs are displayed in the waiting room. (Up) 3. Financial and funding information The Practice has a local Personal Medical Services (PMS) contract with the Shropshire County PCT and the Practice receives money from the PCT according to its contract in exchange for the services it provides for patients. A PMS Contract gives providers of primary care services the scope to find new ways of addressing local as opposed to national problems. The contract requires the Practice to provide a full range of services and lists key Health Process indicators that target particular services and lays down annual improvements that the Practice seeks to make over a three year programme. The progress achieved is monitored by the PCT annually.
(Up) 4. Regular publications and information for the public This information is updated annually and can be accessed by patients both in hard copy and on the practice website. Some information is also published in the practice leaflet which is made available to all patients. Patients are entitled to access their own clinical records and any other personal information held about them and requests for this information should be directed to the Office Manager. (Up) The Practice hopes that patients will have no cause for complaint about the service offered. However there is a practice procedure, approved by the Primary Care Trust, to be followed if a patient feels that a complaint is justified. A leaflet is available from the notice board in reception which explains the system. The Practice prefers written, as opposed to verbal, complaints as a record needs to be kept. If required, the Office Manager will provide assistance in completing the complaint form. The Practice undertakes to respond in writing to a written complaint within one working week of it being received in which the patient will be informed of what action the Practice is taking. One week is usually sufficient time to establish the facts but an extended investigation may be required such as when someone involved in the incident is away from work for some time. The investigation will be carried out by the Practice Manager who will call for the help of a GP partner if the complaint is of a clinical nature. At the conclusion of the procedure, the Practice will make a written response to the complainant in which it will be made clear that in the event of the complainant not being satisfied with the Practice response, the complaint will be directed top the PCT. (Up) 6. Our policies and procedures The Practice has written policies and procedures that are followed by doctors and other staff. The core document is the Standard Operating Procedures (SOPs) which covers the service offered to patients, staff issues such as training, finance, clerical procedures, complaints procedures, administration, health and safety and the maintenance of equipment and buildings. Additionally the Practice complies with other statutory regulations such as data protection, employer's public liability insurance, and Phonographic Performance licence. The practice policy requires all doctors working in the Practice to have professional negligence insurance policies with the Medical Defence Union (MDU) or the Medical Protection Society (MPS). (Up) Any enquiries on this publication scheme should be directed to the Practice Manager. It is the policy of the Practice that subject to other provisions that restrict access to information such as the Data Protection Act, a commitment exists that information should be routinely available to those members of the public who request it. Cost of Information For the most part, we will charge you only for hard copies. Some information is available for free, but for others there may be a charge. The charges will vary according to how information is made available. Charges are as follows: [a) Via the General Practice/Primary Care Trust Website - Free of charge, although any charges for Internet Service provider and personal printing costs would have to be met by the individual. For those without Internet access, a single print-out as on the website would be available by post from the Surgery or by personal application (£1.00). However, requests for multiple printouts, or for archived copies of documents which are no longer accessible or available on the web, may attract a charge for the retrieval, photocopy, postage etc. We will let you know the cost and charges that will have to be paid in advance. We will not provide printouts of other organisations' websites.] b) Leaflets and brochures - Free of charge for leaflets or booklets on, for example, services we offer to the public.
